Birth / Age Certificate (Xerox copy) is to be submitted.

Above 15 years 2 copies of student photo is required, 1 copy photo to be affix with the form.
Birth / Age certificate (Xerox copy) is to be submitted.
Students past activities to be mentioned (if any).
